Liability agreement:

"This application is to obtain a permanent registration card for adult soccer in Idaho. Your registration will be permanent as long as you play and maintain current registration through annual renewal. Applications are automatically accepted unless your previous record of play suggests repeated failure to abide by the rules of the game, in which case additional information may be needed. IF THE TEAM FOR WHICH YOU PLAY BELONGS TO A LEAGUE AFFILIATED WITH THE IDAHO STATE SOCCER ASSOCIATION, THEN YOU MUST BE REGISTERED WITH THIS FORM OR YOUR TEAM MAY FORFEIT ANY SANCTIONED GAME WHICH IT MAY PLAY. Please read all of the instructions and the contents of the form carefully, since accurate completion is necessary for proper validation of your liability insurance. If you have any questions, please call your team representative, manager, or your League registrar.

ALL OF THE INFORMATION IN THIS REGISTRATION IS REQUIRED. This is the minimum information necessary to register you as a player with regards to both USSF and insurance requirements. Please answer ALL questions. If you omit any information the application will be returned to you for resubmission. FEES SUBMITTED WILL NOT BE RETURNED AND YOU MAY BE ASSESSED ADDITIONAL FEES TO REPROCESS. Note that if you are a professional player you must include a copy of your contract or other employment agreement with this form, if applicable. Also note that parental permission form must be completed and a parental permission form must be submitted if the player is under the age of 18. Check with your league registrar for this form.

PLAYER WAIVER: In consideration for being allowed to participate in any way in the U.S. Amateur Soccer Association athletic/sports program, and related events and activities, the undersigned:

1. Agrees that prior to participating, they will inspect the facilities and equipments to be used, and if they believe anything to be unsafe, they will immediately advise their coach, supervisor of such conditions(s) and refuse to participate.

2. Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions or negligence of others, the rule of play, or the condition of the

premises or of any equipment uses. Further that there may be other risks not known to us or not reasonably foreseeable at this time.

3. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.

4. Release, waive, discharge and covenant not to sue the U.S. Amateur Soccer Association, its affiliated clubs, their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable owners and leasers of premises used to conduct the event, all of which are herein after to as releasees, form demands, losses or damages on account of the injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the release or otherwise.

CERTIFICATION: By submitting this registration application I agree to abide by the laws and rules of the game of soccer as promulgated by the International Football Federation (FIFA), the United States Soccer Federation (USSF), the Idaho State Soccer Association (ISSA), and any properly affiliated league or team which may sanction a competition in which I choose to participate. I further agree that failure to abide by these laws and rules may result in the revocation of the right to play granted by the acceptance of this registration. I acknowledge that I assume the risk for any personal injury I sustain before, during or after a game and/or practice, and I will not hold liable any team, club, state association the U.S. Amateur Soccer Association, or the U.S. Soccer Federation.


PARENTAL RELEASE STATEMENT - To be completed by parent or guardian if player is under 18 years of age.

I, (parent/guardian), hereby give my permission for my dependent to register with ISSA and actively participate as

required in all activities, matches, practices, and all other events relating to soccer as it may be sponsored and organized by ISSA or its affiliates. In completing this document, I am aware that USASA (and its affiliates does not seek the services of any player that has not reached the age of eighteen prior to registering with ISSA. I further understand that ISSA does not provide protection, expressed or implied, in any manner with respect to crowd control, acts of nature, condition of playing and/or practice grounds and that any and all bodily injury or damage and/or damage to personal property is the sole responsibility of the player and that ISSA repudiates all claims as may arise as a result of lack of protective personnel or equipment except that ISSA shall have the authority to enforce claims against the player in the event that the player is deemed to have contributed to the damage or loss."
